This episode reveals that the events of “Death Wish” have thrown the Q Continuum into a civil war between individualist and collectivist factions. For reasons that aren’t really explained, Q believes that the solution is for him to have a baby with Janeway.

She refuses and convinces him to instead have a baby with his ex-lover, an apparently female Q.

They do so, and Q asks Janeway to be the godmother.
